A bulging disc is more broad based along the posterior side of the annulus, and a protrusion is more focal, and the extruding annulus material is pushed out a few millimeters further than a bulge. A torn annulus is often present with protrusions and bulges.
Herniation of the Nucleus: Herniation of a nucleus is where the nucleus migrates out of the center of a disc and often through a tear in the annulus, which may or may not be associated with a bulge or protrusion. However, one can have a torn disc without a bulge or protrusion, and this often cannot be diagnosed without a discogram.
